To become a new client you must have your Family Doctor or previous Psychiatrist/ Therapist send a referral over by fax to us at 423-648-9291. The reason for this is to provide the most comprehensive care by having information about any medical conditions or past treatments that might affect our choices for your care. 

In order for your referral to be processed quickly please see below

REQUIRED INFORMATION from Referring Provider

• Reason for referral e.g. need medication adjustment, my doctor does not write the medication I need

• Demographics Current Name, address, working phone numbers and all insurance information

• Copy of insurance card front and back

• Last two office notes

• List of all medications

See the main insurances accepted on our insurance page link. If you do not see your insurance company listed, feel free to call us to check if your provider is on that insurance panel.  It varies from provider to provider with some of the insurance companies.

Please don’t forget to upload your driver’s license or other form of ID along with a copy of your insurance card front and back to the New Patient Packet

Please be patient as once your referral is received it can take up to two weeks to process your information.
